The -2 issue is still not resolved even with the newer update, there are no duplicate slugs but every time a page is edited and updated -2 is added at the end of the slug. Please help with this issue.

But I didnot add the text input field with the ph-prefix div , this is how the form has been working for a very long time, but as soon as I update the plugin the issue shows

Here is the form link on staging site, I have updated to the latest version, here you can see it is giving an input field as well and its overlapping and i am able to type in the text. Before updating it was working fine.
