VFBHi there Josh, Yes, VFB can achieve this. It looks to be just basic fields, what really makes the form would be the use of the Layout, which the Free version has, but the Pro version does have a superior Layout option to narrow it down to mirror the old form the best. Feel free to the use the demo site below of the Pro version to get a feel for it.

vfbHi there Rachel, there are some fields such that have the description input under the field label due to long field inputs such as with the Address field. With the Free version, there are no options for specifically designating a fields description to be above or below. With the Pro version there are options to allow the description to be below or above the input. Feel free to take a look at the demo site for the Pro version to see how it is done – https://demo.vfbpro.com/.
Also, adding space between each field can be achieved via CSS. The following CSS should do the trick.

In reply to: [Visual Form Builder] Change date formatSure thing, so if you have set the Date Format for the Date Field with the following format – dd/mm/yy. Then it should adhere to that format when you select a date from the live form. If it is not, I suspect a possible theme/plugin conflict. Try deactivating all plugins/theme but for VFB Pro and give it a test.
