Hello dgibb, this could be the case, are you able to restore the files with a backup by chance – some host companies keep file backups. There could be a number of other reasons why the submissions have stopped, anything from something changing server side, or in WordPress. Have you consulted with your Host provider to ensure nothing has changed that could prevent submissions?

Hi Billy, the only occurrence of this is when the user clicks more than once, but you say your friend only clicked once. By chance what versions are your PHP and MySQL?
