siteground has a few methods of caching to increase page speed.

The NGINX caching policies they have worked fine with the plugin.

The issue was with the Dynamic Caching that siteground offers. Disabling the Dynamic Caching in the SG Optimizer plugin does the trick.

Thank you for the reply. Indeed it is an issue with the server. I ended up using the MailJet Plugin instead which worked https://en-gb.www.ads-software.com/plugins/mailjet-for-wordpress/

The form inputs should also be wrapped in a div to make them easier to move.

It would also be helpful to have a unique ID for field that is generated – although not vital.

After some more investigation. The JS is not loaded when echo do_shortcode is used. Manually adding these bits of JS allow the form to work but this feels like a bad solution.
