You can add custom required fields, before or after the quiz. I’ve added Full name and email address. Once notified that a user has completed the test I can go to statistics and get the person’s email address.

I was able to put icons inside the div id = access of the 2011 theme. I simply added another div that contains the social media icons. Then in style.css you style the way you want it to look. I used the ideas on this page:

This does limit the amount of pages for navigation but on this particular site the client didn’t want very many.

I took what you had done and added style tags for the UL, and LI To help others with the same problem I create a video tutorial.
