The IP address was 37.59.47.*, which is not within the providers IP address space.
Oops, meant to include that.
I have both set to 5 minutes.
I currently do not throttle any traffic. But I have Login Security Options set to log out after 3 failures on login or forgot password attempts.

The fault was not with this plugin at all.

Can the paid version use an alternate checkout form?
Since my client’s customers can purchase either download items or printed copies we ask of shipping information. Would be great if the free items (always download) could bypass the shipping/billing details and only ask of an email address.
I have a test domain/server setup. If it would be of help in troubleshooting I’d be happy to provide temporary access to it.

In reply to: Sidebars duplicated in page without call to get_sidebarOk found my solution and posting it here in case anyone else runs in to the same problem.
The issue is related to the use of dynamic widgets and themes that automatically place widgets in a sidebar if the user hasn’t defined any for that location.
So for example a sidebar named ‘header_top_left’ would be picked up by the dynamic/default widgets and be placed in the ‘top’ sidebar by a given theme.
So naming convention is important.

Your thought had occurred to me as well and I did go through the other template files to see if they called get_sidebar() with the new sidebar names, but they do not.
I’ve replicated the same issue on a fresh install of WP, no plugins, using the Twenty Ten theme. So I’m fairly certain that it isn’t an Artisteer issue. (But I did post the question there as well.)
On a lark and to test if there are any naming issues I renamed the sidebars using a random string generator. But the name of the sidebar has no impact at all.
