I had the exact same problem of a Ninja Forms form not displaying.
I solved by purging WP Rocket cache.
Could you tell me exactly what setting you changed in WP Rocket so that this does not happen again.

It appeared when the installation of Neve went wrong, certainly a download problem
Sorry if I got a little annoyed, the copy paste answers don’t help much. Especially since I had to rebuild all the header and the footer.

Everything has been going well for several months and it’s the update of Neve that breaks everything but it’s the fault of a plugin and not yours.
I find that unsatisfactory as a way of working.

The extension is in 4.1.2
So, I purged the cache.
I think there is no need to do anything else
