I am having the same problem. I have created the form, added fields and field options, and the form doesn’t show up – just the “send message” button. I have used this plugin to do the contact form on our other site, and it works perfectly – so I set this up on the new site exactly the same way (as far as I can tell), but no luck. I deactivated BP Registration Options for the time being in order to spend time on other issues. I have not tried the fix described at the link above, but you might want to read up on it and see if it would work for you. If so, do post whether or not it was successful! I may want to reactivate the plugin.

I am experiencing a similar issue. Once someone has created an account and received an email with link to activate, they need admin approval to see the rest of the site. But instead, they can log in and see the other pages in the site, but every page has this same kind of error message. Hopefully someone can help out with this. The plugin seems to work well otherwise, but this is key.
