Since one month there is no reply from you, I may consider that the prob has been resolved now.

Just change the width of page plugin. You have used both width and height 30.
The Min. width shall be 180 & Max. 500 whereas the pixel height of the plugin should be Min. 70.

Yes, you have to insert widget first for using shortcode. However, I will enable this feature to use shortcode independently in the next version of this plugin.

You have included three times facebook-jssdk scripts in your blog and therefore the AppIDs are conflicting each other.
