I found out the cause of my problem. Apparently Autoptimize was attempting to minimize a JavaScript file that was being inserted at the theme level,
jquery.expend2.js. As the name implies, it’s a JQuery file, which Autoptimize already has rules for excluding, but the built-in rules did not cover this one. I added it to the exclude list and the FAQ page began displaying normally, header, closing</head>, opening<body>tags and all. I would like to thank @optimizingmatters for the help you gave dealing with this matter.This appears to be an interaction between Autoptimize and our custom theme, which is a significantly modified child theme of Twenty Fifteen, as I, using Health Check’s Troubleshooting Mode, could see this as soon as I had switched back to our custom theme and reenabled only Autoptimize. That said, this does not show up in the parent theme, Twenty Fifteen, so I’m not sure there’s a lot more you could do to help me here. With that in mind, I plan to spend the next couple of hours debugging this custom theme to see where’s its going wrong. I would like to thank you @optimizingmatters for the time spent trying to help debug this, but this looks like a problem on our end.
This may occur on other pages, but so far I’ve only seen it on the FAQ page for this site. The problem does remain when HTML and CSS optimization is off, but goes away immediately when JavaScript optimization is turned off. The FAQ page has a special template of [FAQ] and is generated using the plugin Toolset, however, deactivating this plugin did not cause the header to reappear. I did see some related errors in my error log, but I suspect some of these are from trying to install the beta on a site that already had the stable version of Autoptimize installed.
