It is clearly mentioned in the plugins page that the word template file uses macros. You have a choice not to use this plug-in. Wish you all the best.

Thanks for the feedback. Yes, currently category is not imported as each quiz provider is dealing with categories separately. Some of them are allowing a category to be created and others are not allowing it to be created at the time of quiz creation. Again, importing questions into an existing quiz is not possible in all quiz providers. I need to come-up with an idea to overcome these problems with quiz various quiz providers.

Download the plugin to desktop or laptop, and unzip it. Inside the unzipped folder you can find the word template file.

Template is inside plugin zip file.

I think you need to update import/export plugin to include question_answers.
