Found it. It was a setting in iThemes Security to not allow PHP execution in plugins. Thanks for your assistance!

In further troubleshooting I found that if I turn off Guest Mode the problem is solved.

Resetting to default settings makes it work.

On our form, the PayPal Checkout shows 3 buttons (confusing to the users).
PayPal Checkout
Pay Later
Debit of Credit Card

As far as the processing issue which is intermittent, the form is completed and the user is taken to PayPal and completes the payment portion of the process and then is shown a blank screen with the spinning icon and it just hangs at that point indefinitely. The transaction is never recorded in the Forminator database and no confirmation emails are sent.

Let me know what additional information you need and if you need admin access to the website.
