I’m testing it on WP 3.9 and it works perfectly. Maybe there is any conflict with your theme or any other plugin.

Plus if you are using PHP5 it is possible that if fails working. In this case, try to comment the linefunction DojoDigitalHideTitle(){ $this->__construct(); }

Actually, I have no problem when WPML is disabled. I guess that WPML is the cause of this problem. It should have some kind of incompatibility or bug with this extension.
The strange thing is that I encounter this problem only when I modify 2 pages. With the other pages there is no problem.Firstly I thought that the theme was the cause of the problem because the template was changed but now I think it is a problem specific to WPML.

@seamoose37 – 0777 matches with read/write/execution permissions on Linux (for all users). Previously the permissions were 0444 (read only for all users). I didn’t read the code but it seems that the captcha files were not regenerated when submitting the form.
