This issue is still ongoing, and I’m using 5.6.2. What’s more, I don’t want the checkbox to automatically be checked, which seems to be another problem.

I don’t have a half a week to wait for your response. Since that is the case, I’ve had to come up with an alternative method.

I really wish this worked for me.

I’ve changed themes, disabled other plugins but nothing has changed. Anyone else have a solution to this problem?

I already enabled the Font Awesome 4 and there still are no checkboxes on the form displaying.

If the plugin developer isn’t going to respond, obviously this isn’t going anywhere. I’m currently looking at Recent Posts Widget With Thumbnails. I’ve used it before and am thinking of going back to it, which is simple to use but a pain to format the layouts.
