Additional info: I found out from our content editors that this plugin was installed and working fine, but they started getting errors and removed the plugin. After they re-installed the plugin they began getting the errors that show up in the screenshots in my original post in this thread.They have tried installing both via the gallery and manually uploading via FTP.
@royjemee Additional details regarding the delete process:
Once I have activated your plugin on one of our multi-sites, the error message displays and I can no longer access the Plugins section of that multi-site. Which means I cannot deactivate the plugin. Since the Network Admin Level won’t let me delete a plugin that is active for a multi-site, I take the following steps:
1. Rename the master-addons folder.
2. Now I can delete the plugin reference from the Network Admin Level.I believe I tried deleting via some other scenarios as well, but still end up with the same error message each time I re-install and activate for the multi-site.

Thanks for your response. I have already tried deleting the plugin and reinstalling and I end up with the same error messages.
After deleting and re-installing, everything seems fine until I activate the plugin on one of our multi-sites. Then I get the error shown in the screens above. To be clear, I am not activating the plugin at the Network Admin Level (Network Activate).
