Hi Andrew,
Since my query I have palyed around further and managed to achieve what I was after. I had originally tried creating the new form associated with the Account Tab using the registration option. Tried it using the Profile option and its all come together. Thanks for your suggestion.Regards

?Thanks for that. That explains it. I didn’t realize it went down the length of the column before moving to the next column on the right. have restructured my form accordingly.

I will activate the plugins one by on e to see which is conflicting.

Wordpress version is 4.8.2
