Thanks for getting in touch about this. To add an additional checkbox custom field to your WordPress form, you have a few different options:
1) Within the plugin you can choose “Create New Field” and click the add button: https://cm.d.pr/N4n8Dr
Give your field a name, and make it a required “Multi Select Many” field with only one option: https://cm.d.pr/fmXkKe2) Alternatively, you could first create the “Multiple options (can select many)” custom field in Campaign Monitor first. Then when creating your WordPress form, you should be able to select?that custom field from the same “Create New Field” dropdown, as seen here: https://cm.d.pr/5iiDe6
Note that you’ll have to select the Client and List first in order for the custom field options to appear.
If you’re not seeing your custom field showing up in that dropdown, could you please try Deactivating the plugin under the WordPress plugins menu, and then re-activating it, to see if that resolves the issue?

No worries at all.
Could you please try reconnecting the plugin?
There should be a button under the settings page to disconnect the plugin and then connect again. Your form should still be there after reconnecting the plugin.

I see that you are attempting to get the shortcode for the “bar” form type. The “bar” type is a form that overlays on your site.
You can select which page(s) it will appear, as indicated by the dropdown “Page(s) this form appears on”.
For your reference, the shortcode is available for the “button” and “embedded” form types.

– Are you currently running the latest version of WordPress? What is the version number?
– What is your PHP number?
– Can you confirm which page in your WordPress Admin, you looking for the shortcode?
– Is the ‘Form Embed Code’ field blank when editing your form?

Based on the error message you received, I believe this is going on:
There is a known issue that if you were running anything prior to 2.6 of our plugin, you will not be able to successfully update to 2.8 of the plugin. About three years ago, we improved how the plugin saves forms in the WordPress database and the 2.6 update is essential to ensure subsequent successful updates.
To solve this issue, please follow these steps:
1. Go to your WordPress Admin and disable our plugin.
2. Download the 2.6.2 version from here https://www.ads-software.com/plugins/forms-for-campaign-monitor/advanced/ (you will find it in the select menu at the end of the page).
3. Go to WordPress Admin / Plugins / Add Plugins, and upload the file you downloaded in the previous step. Then enable the plugin.
4. Open the Campaign Monitor tab in your WordPress Admin, and follow the instructions to update your existing forms.
5. Only when you complete the previous step, update the plugin to 2.8.0 from your list of pending updates in the Plugins sectionPlease let us know if this works for you.

1. did you reconnect the plugin to your Campaign Monitor account once you updated the version to 2.8
2. Were you originally updating from 2.6 to 2.8 or did you have a version previous to 2.6 before you attempted the update to 2.8?If yes to both, please confirm the version of PHP and WordPress you are running and any further details you have about the error.
