This has not been resolved, and an update released. If you update your plugin you shouldn’t see this problem.
Thanks for bringing this to our attention! This has now been fixed and should no longer be an issue.

This is the first we are hearing about this. We have ~2400 downloads in the last 4 months and no one has complained about this. I also just tested the plugin and it is working well for me.

We provided a few different ways to customize your form (6 form types, flexible field types) but changing the order of fields is not part of this release. I will keep your idea in mind for any future iterations. Thank you!

Here are the WordPress requirements which state you need to run
– PHP version 5.6 or greater
– MySQL version 5.6 or greaterIf you upgrade to these versions and still encounter any issues please let me know.
