This plugin might help narrow it down. It lets you see what form content has been submitted. That should at least let you know that the form is working.

Do you have the proper email modules installed and running for your server?

Firewall or anti-virus?

Why do you need to add the ‘id’ attribute? Are you trying to use jQuery for something? If so, you can use the ‘name’ attribute instead with something like: $(“input[name=nameGoesHere]”).

It sounds like you have it setup correctly. Is it spelled correctly? That’s always the easiest thing to start with. I’ve been using the plugin for a couple years without issue.

I haven’t used this before, but could this work for you?

You need to also include the .js files needed for the plugin. The shortcode doesn’t add those in.

It’s not a WordPress site.

What did you add in your .htaccess file?

Does the issue happen when the widget is removed? plugin disabled?

If it’s on pages without the widget, then it sounds like a more general problem to the .htaccess file.

Maybe remove what you added to .htaccess and just install Better WP Security

I’ve been using that plugin for a couple years now and haven’t had any mysterious posts appear on my sites. My guess is that someone/something is messing with your site. I would think that if you didn’t create the post that it would be safe to move to Trash. If nothing breaks on the site without it, Permanently Delete it.

I would also install a security plugin such as WP Better Security.

I am using it without any issues. I have it Network Activated.
