Are you running the latest version of Formidable Forms? Our developers very recently fixed a bug related to the spam filter on file upload fields, so updating to version 4.09.05 will likely resolve this issue for you.
If your plugin is already up to date but you continue to receive new reports about this, please open a support ticket and we will be happy to investigate further.

In the form builder, simply click on your checkbox field to see its Field Options and then click on your chosen checkbox to select it.
When a checkbox is selected in the field options, it becomes selected in the form by default.

Unfortunately no, it wouldn’t be possible to build what you’re describing using the built-in features of our plugin. You might be able to achieve what you’re looking for with extensive customization, but I’m afraid that would be beyond the scope of our support.

Sorry to hear you’re having trouble with this! The issues you’re describing are most likely being caused by a conflict with your site’s theme or another plugin you’re running.
I recommend running through these troubleshooting steps to find the source of the conflict:
If the issues go away under those circumstances, you can add each item back in, one at a time, until you find the culprit.

You can retrieve values stored in an ACF field using the post_meta key shortcode in your Formidable field. Here is the method you would use to do that:

Some of the more advanced ACF fields cannot be mapped with Formidable, but a contact name (text) and email field should be fine. You can find a list of all the Formidable field types that map easily to ACF fields here:

There is no limit on the number of entries you can export to CSV/XML in Formidable Lite, so it sounds like this issue was caused by a problem with your configuration or perhaps a bug.
Please submit a new support ticket including all the details of the problem you encountered with your export file and our team will be happy to investigate further.

Unfortunately, there is currently no built-in way to manage PDFs in bulk like that with Formidable.

Sorry to hear that didn’t work for you! Unfortunately that’s the only workaround available for the Lite version of Formidable.
If you switch to the default WordPress theme, does that resolve the issue? If so, that means your particular theme’s styling cannot be overwritten by our plugin so you may want to consider using a different theme.
For advanced support with styling conflicts, you would need to upgrade to Formidable Pro.

If the default Formidable style isn’t being applied to your form even after you select it in Settings → Styling & Buttons → Style Template, then it’s likely that your WP theme is overriding the CSS.
