Changing to the default twenty twenty three WordPress theme causes all fields to become visible. So there is a theme conflict.

I have not tried to switch themes for fear of messing the formatting such as it is.
I will say, I am fairly sure that the conflict is with the theme though. I have tried the same and similar forms on other sites and they work fine.
How difficult is restoring a site after switching back and forth between themes?
I went through the plug ins. It didn’t seem to make a difference.

No you are not missing anything obvious. The problem is that fields (usually about the fourth one in) are not displaying. The work around – which is used on the form you tested – is to put in a double of the missing field. I have added some screenshots to illustrate:

If the missing field is marked as “Required” the form throws an error. Does this make sense? I’m not sure I am explaining it very well. The jpegs are in the same folder with the JSON files:

There are two exports, both about the same but each one has a different doubled field. The “Required” twin is the one that is displaying for me. The other is not.

Um…happy to, but (and this is a rather obvious total beginner question) what is the best way to share a file here? I don’t see an attach option, so do I share a Google Drive or Dropbox folder?

I think I have localized the source of the Error. I’m not sure why I did not notice this before, but in the course of trying to test recreating forms out today I realized that one of the seven fields on the original form in question is not being displayed. The omission happens on both the edit-preview and on a live site contact page. Since all of the fields were set to “required”, the form couldn’t be finished.
I have tested recreating the original form in several ways. If I create a new form, add one field at a time, and test the form (add field, save, preview, test, repeat) the problem seems to show up about the fourth field in. This is consistent with the missing field in the original form.
The work around I found is to simply duplicate the non-displaying field, which, as long as the non-displaying field isn’t set to “required”, works. However, this seems like a less than robust solution. I’ve done a bit of web searching and this doesn’t appear to be problem anyone talks about.
I hope this provides a bit of insight.
Also, I asked the tech help for the theme, and they say there is not a optimizer of any kind built in to the theme…if that makes a difference.

Unfortunately, shutting of antispam does not seem to make a difference. reCaptcha is already active.

Actually, there are no optimization plug ins installed. So, I don’t think the documentation applies. I’ve created other forms for other sites, but this is the first time I’ve gotten the “Form has not been submitted, please see the errors below” message.

Actually, wait a minute. When I go to test the form I get a “Form has not been submitted, please see the errors below”, but there are no errors shown below. Can you help on this, or should I start a new thread?

Just out of curiosity, was there someplace in the theme’s code that you could see the conflict? Or was it more that since it wasn’t the plug ins, it had to be a theme conflict and the radio code is a standard try fix?
