Could you explain a little further what you are trying to accomplish?
I think if I understand correctly, you would like to access the plugin form field “Your Name” from within ClickUp? If this is the case, unfortunately, you’ll only be able to insert it into the description of the task being submitted through the plugin. At least with the current feature set.
