Are you sure this is a W3 Total Cache issue? Meaning, when you deactivate the plugin, the issue goes away?

Also, do you have the “Don’t set cookies for static files” Browser Cache option enabled?

Try disabling gzip in under Browser Cache.

Origin push CDNs are a pain and notoriously hard to troubleshoot, you’ll save yourself a lot of time and frustration if you switch to an origin pull CDN product like MaxCDN.

You have to enable Browser Cache and also navigate to the settings page and configure it, it’s not automatic.

I also tried adding the below code to the top of my .htaccess file but this didn’t work either.

Not necessary, you need to remove that code.

URIs with a query string are not cached by default when using Disk Enhanced page cache.
