Yes, Smush free version will optimize jpeg format.

After visiting your page on different browsers I could not reproduce the issue, the flag looks good:
Please try as incognito or private mode on Egde for Win or Safari for Mac.

As an alternative, and for testing could you downgrade Forminator to the previous version? You can download it from this link:

If you downgrade please let us know if the issue went fixed.

What you described and after checking your screenshot the issue seems to be related to an assets optimization or a plugin blocking Forminator JavaScript assets, would you let us know if you are using an asset optimization or caching plugin on your sites? Also, are any error in the browser console?
If you are using any of the plugins, could you temporarily deactivate it an see if the issue persists? In case the issue persists, we would suggest performing a conflict test, kindly follow this guide for it: https://wpmudev.com/docs/getting-started/getting-support/#conflict-test.
We stongly suggest performing a conflict test on a staging site or ran a backup before perform the test.

There is a known thing when it comes to forms using a lot of Conditionals specially when using it in fields, but, we would like to reproduce the same on our side.
Could you export your form using the guide on this link: https://wpmudev.com/docs/wpmu-dev-plugins/forminator/#import-export? Once exported, kindly use a Cloud service like Google Drive, pastebin.com, or similar to share it. If you have sensitive data, please duplicate the form, remove it, and export the form.

After visiting your site I noticed there is an issue with mixed content:
That’s why Forminator style file is not loading.
Here are some suggestions to fix this:
– Using https://www.ads-software.com/plugins/ssl-insecure-content-fixer/ plugin
– Check with your hosting provider if they can replace HTTP with HTTPS URLs on your site database.
– Using https://www.ads-software.com/plugins/better-search/ to replace https:// with https://We strongly suggest you taking a backup before performing any of the suggestions above.

After performing a test on my site I could not reproduce the issue. I am getting the message set:

This is a known issue with the Phone field when there is no validation, an improvement is planned but we are unable to provide an ETA about when this will be released.
In such cases, you can enable the Phone Validation:
Still, this doesn’t allow only numbers it will validate the phone numbers according to the Country and it does contain a format:
If you want to only allow numbers you might need to use a different field, which is the Number field:
You can limit the char for this field as well and it will not allow any chars besides number.
