Your site HAS been hacked. This is an old WordPress hack that I found as far back as 2013. It’s sending information from your database to https://45.55.143.25/mev.php (that’s the URL hidden in the “Token” portion of the URL. You should have your site evaluated for further breaches as soon as you are able.

We’re looking into this now. Our support team cannot replicate the issue locally and we did test and this issue was not revealed. We’re trying to determine the cause now.

This is likely being caused by your Layout and Styles extension being out of date. The newest version should correct this issue.

Kris, I don’t see any support tickets from you in our system…did you try to reach out to us at all? We have documentation that addresses the issue you are likely experiencing (though it should definitely not be happening with only 7 fields in your form and only the core plugin installed). simplyy’s article is the fix for that issue. In Ninja Forms 2.9 coming out sometime later this month (or early next month) we have an idea for a way to resolve this issue a little bit better than the above documentation.

Usually this issue is caused by an installed plugin which is modifying the admin menus if your site.
