I wish I could shake your hand in person, but we’ll have to make it virtual instead. I really am grateful to you for taking the time to look at my issue and suggest this fix.

For some weird reason, my computer wasn’t saving the corrections I made, so the MAIL and FORM fields were out of sync. I would not have realized this without your answer.

The form is working now and I am terrifically relieved. I can only say THANKS! I will pass along the good deed by helping someone and make a deposit in your karma account. Also thanks to periphvision for your diagnosis — I really appreciate it.

periphvision — You get the international good citizen award! I really appreciate your taking the time to look at this. Thanks a bunch. I tried giving separate identifiers to the tags in the form interface and the mail interface, as you suggested. I made each item correspond to the other. But unfortunately it’s still not working. A few fields are OK but the others are bonkers. I’m under the gun to finalize the form and can’t figure out what I’m doing wrong.

I figured out a work-around. Just created a button and saved as a jpg. Inserted it and it seems to be working. Thanks.

Still cannot get the SUBMIT button to appear. Is the developer available? Would really appreciate a hand with this. Thanks very much!
