You can fill wp-config.php manually, but this cannot be the cause of your problem. DB name, username and password you can get from your provider. Username and password might be the same as your FTP 9or whatever) login info, but not necessary. Anyway, this info comes from your provider.
Umm.. Maybe.. You can try to chmod all WP directories to 666 (not that it’s safe, but it can solve the “forbidden” problem). That can be done easily using your FTP, SCP program or whatever you use.

You have at least one extra closing div tag, obviously this is the reason why your footer is not on the bottom of the page. You have messed up something there.

In reply to: Cannot select databaseUmm.. You shouldn’t use hyphens in database and table names. That can cause problems. Maybe not in this particular case, however – better without hyphens.
