I am not an expert, BUT, did you check to make sure you clicked include date entries when exporting? you can choose just the form or the data, views etc as well. I had better luck exporting each element, data, forms, views separately.
also, when importing using csv into Formidable the imports will usually fail if you have more than 500 lines… I usually cut my list into shorter chunks to avoid this.
